Job Summary

We are expanding our programs and services and invite motivated, confident, and client-focused individuals to join our team. The Support Worker will provide direct, individualized support to adults with cognitive disabilities participating in the Supported Independent Living Programs. Support is delivered in a respectful, person-centred, and strengths-based manner to assist individuals in achieving greater independence and community inclusion.

Reporting to the Manager of Supported Independent Living Programs, the Support Worker will provide support in areas such as daily living skills, health and wellness, budgeting, recreation, community connection, and access to local resources. The successful candidate will be adaptable, well-organized, and demonstrate strong interpersonal and communication skills.

Responsibilities

  • Develop and maintain supportive relationships with individuals to meet their personal goals.
  • Assist individuals in accessing services and resources that promote health, independence, and community involvement.
  • Provide support and intervention in accordance with person-centred plans, in collaboration with Community Living Services Delivery (CLSD) staff.
  • Collaborate with external professionals and agencies to deliver coordinated, quality care.
  • Maintain accurate documentation and client records as per agency standards.
  • Perform other duties as assigned.

Qualifications

  • Post-secondary education in social sciences, human services, or a related field, and/or experience working with adults with cognitive disabilities (preferred).
  • Understanding of person-centered planning approaches.
  • Conflict resolution and crisis intervention skills.
  • Effective interpersonal, written, and verbal communication skills.
  • Ability to provide confidential, culturally sensitive, and respectful support services.
  • Valid Criminal Record Check with Vulnerable Sector Query.
  • Valid Class 5 driver’s license.
  • Use of personal vehicle with appropriate insurance (mileage compensation provided).
  • Ability and willingness to work flexible hours, including evenings and weekends as needed.
